THE APMH PROGRAM
Review the provider and practice eligibility requirements and the fee structure for accreditation by APMH.
Provider Eligibility

Pediatric providers that hold a current, unrestricted license as a Doctor of Medicine [MD], doctor of osteopathy [DO], advanced practice registered nurse [APRN], or physician assistant [PA].

Board certification with the American Board of Pediatrics [ABP], American Board of Family Physicians [ABFP], or American College Osteopathic Pediatricians is required or in progress with anticipated completion within one year.

Advanced practice registered nurse [APRN] or physician assistant [PA] owned practices with proper physician oversight, per state laws.
Practice Eligibility
01
Primary care pediatric practices.

Fully licensed and compliant with all state practice requirements.

Single site, defined as any one [1] location.

Multisite, defined as two [2] or more locations that are within the same organization.

Multisite practices must have the following:
* Same TIN
* Share an EMR and other utilized
systems/programs
* Allow patients to visit any practice location.

Technical Requirements: The practice must have knowledge of basic computer functions.

The practice must have a functioning document scanner (a copier/scanner or a mobile scanner app).

The practice must have the ability to navigate virtual platforms and maintain their EMR.

The practice must have the ability to participate in virtual / web-based meetings (for example, via Zoom.
Fee Structure
Practices seeking patient centered medical home accreditation for the first time will be responsible for paying fees to The Alliance for Pediatric Medical Homes for the review and accreditation process. The fees are outlined below. APMH reserves the right to review the entire clinician panel.
Per clinician fee initial accreditation
- * 1-5 clinician(s) – $600
- * 6-10 clinicians – $275
- * 11-20 clinicians – $125
- * 21+ clinicians – $75
Per clinician fee annual maintenance
- * 1-5 clinician(s) – $150
- * 6-10 clinicians – $125
- * 11-20 clinicians – $31.25
- * 21+ clinicians – $18.75
